When anomalies are detected in process data, a case file is created by Tesla Solutions Engineers. This case file may be marked for investigation with a risk based priority. Often anomalies can be explained by upsets reported by operations. In some instances anomalies are a precursors to equipment failure. When equipment fails and is recovered, a Root Cause Failure Assessment is typically conducted and the findings from the assessment are used to refine a database of known failures and their digital signature.
Tesla Solutions can participate in all stages of the anomaly life cycle, but we play a unique role within the the initial and final stages. Case file life cycle begins with the initial anomaly detection and carries on through to the RCFA and then capturing lessons learned for future detection.
The life cycle of an anomaly can range from a few hours or days on a plant that is easily accessible to several years for remote installations such as Subsea or the Arctic. Longer wave case files tend to be left open and forgotten due to the turn over of people within the organizations. For this reason it is imperative that a management system and process are established to prompt new users to check if an anomaly case file is open and available to be built upon or closed out when new information is gathered.
